Subject: 2008 Year-End Survey

As we complete the BPFA summer fastpitch season, we are asking each family in our organization to offer us your feedback regarding your experiences associated with the season. Attached you will find a survey which we are asking you to complete and return to us. Our goal is to review each and every survey and use your feedback to improve our program in every way we possibly can. Your feedback is a vital and necessary tool in this process! We ask that you sit down with your child and go through the survey together. Parent and player feedback is equally important in this process.

The attached survey is designed to gather feedback on our coaches and our program as a whole. Please complete the entire survey as honestly and frankly as possible. We are open to both positive feedback regarding aspects of our program that have met your needs, and constructive criticism for aspects of our program that may not have met your expectations. The survey has two basic sections. The first section is designed for you to circle a number which corresponds to your level of satisfaction with the question or statement you have read. Families on teams with multiple coaches are encouraged to complete the coach feedback section (questions 1-17) for each coach associated with your team. Otherwise complete the front section on the head coach and make written comments on all of your coaches on the back page. The second section (back page) is designed for you to offer specific verbatim feedback on various aspects of our fastpitch program.

We ask that you please take the time to complete the survey and return it one of two ways. You can give it to JoAnn Martinson in person, email it to mjmartinson4@comcast.net or mail it to the address listed on the survey. Thank you in advance for your consideration in this matter!
